Care Coordination is a service offered to persons receiving Medicaid, Managed Medicaid or Medicaid/Medicare.
The goal of Care Coordination is to improve the overall health and well being of the participant by connecting them with needed services.
Eligibility requires two or more chronic health conditions or one of the following:
